The Embark Care Case manager acts as the principal agent in the supervision of field staff and overall client care to ensure that daily operations are performed at a high satisfactory level. The purpose of this position is to enhance the quality of service provided, maximize satisfaction, increase retention and promote cost effectiveness. This position will be accountable for overseeing the full client care cycle including: assessing, planning, implementing, monitoring, hiring of qualified personnel, scheduling, and evaluating actions required to meet the client’s needs.
Qualifications:
Required Graduate of accredited High School. Required Minimum of two (2) years’ experience in health or home care related positions. Must have the ability to work with and motivate people, possess effective written and verbal communications skills, have a thorough knowledge of agency functions and processes, must have a flexible schedule, use work hours efficiently, and be emotionally stable.
Transportation:
Reliable transportation with proof of current auto liability insurance.
Criminal History:
Must agree to and pass a criminal history check and Employee Misconduct Registry check.
Environmental and Working Conditions:
Perform duties in an office environment during agency operating hours. Must always be well groomed and dressed in appropriate attire. Ability to work flexible schedule as the need arises for after hours operational policies and procedures such as EPRP, etc.
Physical and Mental Effort:
Prolonged sitting required with ability to operate office equipment which may include heavy lifting, bending and standing. Ability to travel for client assessments/visits. Requires ability to cope and work under stressful situations in a calm and courteous manner at all times. Requires working under some stressful conditions to meet deadlines and to make quick decisions and resource acquisition; meet client/family individualized needs.
General Duties and Responsibilities:
1. Plans, organizes, administers, and coordinates agency Client/Personnel functions
2. Serves as liaison among the professional personnel and field staff.
3. Reviews with care team, all employee and client information to ensure case placement of qualified, competent, and reliable personnel.
4. Evaluates environmental/working conditions of field employees during home visits, conveys any concerns to Management.
5. Scheduling, supervision and orientation of caregivers as needed.
6. Maintains good standing in the community and attends public events as needed
7. Oversee Start of Cares
8. Oversee Client Discharges
9. Chart updates and documentation as needed
10. Manage collection of Weekly task reports and mileage confirmation
11. Ensure Incident and Infection reports are reported to Administration in a timely manner
12. Ensure Client concerns and Caregiver concerns are addressed and reported to Administration in a timely manner
13. Reviews on-call log to ensure all messages are handled in a timely manner
14. Documents routine communication with employees and clients to ensure the quality of services being provided
15. Available during agency’s operation hours unless on paid time off or approved requested days off
16. Returns all phone calls transmitted by the phone within appropriate time, if not, documents reason call was not returned by next business day
17. Oversees scheduling of unannounced visits to clients to ensure the quality of our services and caregivers are performing as anticipated
18. Responds promptly and courteously to client and employee complaints and inquiries. Refers appropriately to Administration
19. Must be available to work a client case if the need arises
20. Maintains accurate and current list of clients in facilities
21. Completes updates on spreadsheet and in scheduling system for Starts and Discharges
22. Participates in after hours on-call rotation.
23. Demonstrates in-depth knowledge of, and ensures compliance with state, local, and federal laws relating to all the operations of the Agency (includes in-office operations, wage and hour regulations, and EEO)
24. Assistance in all tasks delegated by Administrator/Alt. Administrator as related to agency functions
