Case Manager

Job Title: Case Manager – Community Support Services
Location: Port Angeles, WA (On-site only)
Job Type: Full-time
Schedule: Monday–Friday, 40 hours/week
Pay: $24 to $27 per hour (DOE/DOQ)

About the Role

We are seeking a compassionate and dedicated Case Manager to join our Community Support Services team. You will provide case management services to adults in both office and community settings, helping them achieve goals outlined in their Individual Treatment Plans. This role is recovery-oriented, person-centered, and focused on empowering individuals through support and advocacy.

Responsibilities

• Develop and implement individualized service plans.
• Conduct assessments of clients’ emotional, physical, social, and vocational needs.
• Provide intensive case management and supportive counseling.
• Assist clients in accessing basic needs and achieving educational/vocational goals.
• Advocate for clients with community agencies.
• Maintain accurate documentation in electronic medical records.

Qualifications

• Bachelor’s degree in Behavioral Science or related field
• Two years of experience in case management or clinical services
• Valid driver’s license and insurable driving record
• Ability to pass a drug test and background check
• Proof of required vaccinations (MMR, Hep B, Tdap, flu)
• Agency Affiliated Counselor Registration credential (assistance provided)

Preferred: Experience working with individuals affected by behavioral health challenges.
