Case Managers work directly with clients in a structured environment, providing consistent support and coordination alongside our multidisciplinary team.

Position Overview

We are seeking a compassionate and organized Case Manager to join our team. This is an onsite role where you will meet with clients at one of our Vivia Health clinics, assisting them with treatment planning, resource connection, and skills development.

The ideal candidate is empathetic, detail-oriented, and committed to supporting individuals living with mental health challenges. You’ll collaborate closely with therapists, medical providers, and administrators to ensure clients receive the care and resources they need.

Key Responsibilities

  • Provide direct case management services to clients in an outpatient clinic setting.
  • Conduct intake assessments, develop individualized service plans, and monitor progress.
  • Assist clients in accessing benefits, housing, medical care, employment resources, and community supports.
  • Facilitate coordination of services across internal providers and external agencies.
  • Maintain accurate and timely documentation in compliance with Medicaid and agency requirements.
  • Support clients in building life skills, promoting recovery, and navigating challenges.
  • Participate in team meetings, case consultations, and clinical supervision.

Qualifications

Required:

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field.
  • At least 2 years of experience working with individuals with mental health needs.
  • Strong communication and interpersonal skills.
  • Ability to manage multiple cases and prioritize effectively.
  • Proficiency with electronic health records and documentation systems.

Preferred:

  • IM+CANS certification (Illinois Medicaid – Community and Supports Assessment).
  • Experience in outpatient or clinic-based mental health settings.
  • Familiarity with Medicaid documentation and behavioral health service coordination.
  • Bilingual in Spanish or other languages is a strong plus.

Pay: $22.00 - $26.00 per hour
