The W Group Assisted Living Communities is seeking a compassionate and organized Bilingual Case Manager to join our team at Bronxwood Assisted Living Facility.

This role is ideal for someone who is passionate about supporting residents, coordinating services, and working closely with families, providers, and the interdisciplinary team to ensure quality care and resident satisfaction.

Responsibilities:

• Communicate with residents, families, and care providers

• Coordinate admissions and re-admissions

• Support resident service planning and care coordination

• Maintain accurate and timely documentation

• Work closely with the interdisciplinary team

• Uphold resident rights and facility standards

Qualifications:

• Fluent in English and Spanish

• At least 1 year of Case Management experience required

• Experience in assisted living, long-term care, nursing home, or hospital setting preferred

• ALIS experience is a plus

• Strong communication, organizational, and documentation skills

• Compassionate, professional, and team-oriented
