JOB SUMMARY
Under the supervision of the Program Supervisor, the Case Manager is responsible for the ongoing development of recovery plans of action, accessing community resources, monitoring clinical engagement of treatment and ensuring recovery activities and responsibilities are completed by participants.
ESSENTIAL DUTIES
- Researches and locates community and recovery resources for all participants.
- Assists in the ongoing development of recovery plans of action.
- Monitors the engagement of the clinical treatment of the participant with the multidisciplinary team.
- Conducts life skills and recovery meetings periodically based on qualifications.
- Independently manages their caseload and documents all services provided in SAMMS and/or additional trackers as needed.
- Performs contingency management planning and utilization.
- Reviews weekly recovery activities, plans, and goals and makes recommendations.
- Attends and participates in assigned meetings.
- Participates in team meetings, agency meetings and agency fundraising efforts.
- Creates and disseminates communication for participants.
- Maintains long-term stable recovery.
- Discloses, as appropriate, being in recovery from a mental health, substance use or addiction diagnosis.
- Shares his/her own story of recovery as an inspiration to others.
- Obtains and maintains state certification.
- Takes direction, is prompt, takes initiative, completes tasks as assigned, and is able to work effectively as part of a team that includes community stakeholders.
- Articulates effectively and has good communication skills.
- Agrees to adhere to the Code of Ethics.
- Performs other tasks as assigned.
REQUIRED SKILLS/ABILITIES
- Has at minimum a basic understanding of: the concept of recovery as it relates to behavioral health, Peer support (Recovery Coaching), and Wellness and recovery planning.
- Good oral and written communication skills.
- Commitment to ongoing professional development and training.
- Willingness to work in the community and in the office.
- Bi-lingual (English/Spanish) preferred.
- Flexible to adapt to schedules and workload to the needs and demands of the program.
- Possession of a valid Illinois motor vehicle operator’s license and willingness to use personal vehicle in course of employment.
- H.A.S. requires all staff to work onsite.
EDUCATION & EXPERIENCE
- Must possess a High School Diploma or its equivalent.
- Must have a minimum of two (2) years of sustained sobriety. Three (3) years preferred.
- Must have CRSS and CPRS Certification from ICB/IAODAPCA or obtain within 1 year from date of hire.
- CADC certification is preferred but not required.
PHYSICAL REQUIREMENTS
Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Repetitive motion. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
