Case Manager

Title: Case Manager (CM) I, II, III

Supervisor: Program Director

Summary of Job Description:

Help to develop and implement treatment plan goals and targets for Members in collaboration with the multi-disciplinary team. Participate in crisis management, program activities, and provide direct oversight for all members. The CM works closely with members on goal attainment and supports members as they transition into less structured living arrangements.

Specific Responsibilities:

  • Collaborate with admissions to support the admission and acclimationof new Members into the program setting.
  • Complete Incident Reportsin accordance withprocesses outlined by the Managed Care Organization (MCO), State and Project Transition/ PCS Mental HealthPolicy and Process.
  • Collaborate with members of the multidisciplinary team to develop treatment plans foridentifiedMembers.
  • CompleteweeklyindividualCase Managementsessions with Members anddocumentin DAP format.
  • Facilitate/ Co-facilitate all groups as assigned and complete documentation.
  • Support with transporting Membersto appointmentsper the directive of the Program Director.
  • Support with observation of Memberdrug and alcoholscreenings and document observations(if applicable).
  • Participate in phone coaching/ crisis management process, which includes (but is not limited to):
  • Orienting Member and family to phone coaching process.
  • Carry and respond to coaching phone calls on days assigned, including being available to go onsite for emergenciesduring hours when there is no or minimal staff coverage.
  • Provide coverage for staff callouts.
  • Following incident reporting and documentation process for all on-call emergencies.
  • Support Members withlinksto services within the community.
  • Complete family contacts for assigned Members per requirements.
  • Ensure all Members have an emergency contact documented.
  • Ensure all consents are completed and updated yearly.
  • Review all Member’s Advance Directives annually.
  • Support Members with activities of daily living including (but not limited to)the following:
  • Daily oversight and coaching of daily living activities.
  • Observing Member self-administration of medication.
  • Support with the development of socialization/ communication skills, conflict resolution, de-escalation skills, andutilizationof crisissupports.
  • Support with meal planning, menu development, foodshoppingand meal preparation.
  • Encourage and support health and home safetypractices.
  • Support with navigation of publictransportation services.
  • Completion of residential roving and checklist to ensure safety.
  • Support with Discharge Member Follow-Up.
  • Complete continuing educational requirements as aligned with regulatory compliance.
  • Attendindividualsupervision sessionsand program meetingsas scheduledbydirect supervisor.
  • Ensure documentation ofall Memberengagementsiscompletedwithin 24 hours.
  • Other duties as assigned by the Program Director

Additional Performance Expectations:

  • Participate inmultidisciplinarytreatment team and will support and implement interventions and directives as directed by the Team.
  • Always demonstrate compassion and concern when supporting aMemberthrough embracing Project Transition/ PCS Mental Health’s Mission and Core Values.
  • Approach Member engagement from a non-judgmental stance understanding that aMember’sbehavior is driven by experience, which may include trauma.
  • Treat and speak to Members with supportive kindness even when aMemberdemonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyles.
  • Seekappropriate support, consultation with Clinician or Psychiatrist(if applicable), inconjunction with the Program Director or obtain supervision, when they are uncertain about how to respond or support aMembereffectively.
  • Report back to the TreatmentTeamany observations of Member behavior that suggests Member may needadditionaltreatment interventions and/or support.
  • Engage with all external parties/ individuals with professionalism and with a positive customer service approach, understanding that they are always representing the organization.
  • An understanding ofanagreement to value the concepts of a Trauma Informed workplace.

Physical Requirement:

This position requires the ability to perform physically demanding tasks, including standing, walking, bending, and lifting up to 20 lbs. An essential function of this role involves the application of physical restraint techniques in emergency situations to prevent immediate harm to the Members or others, requiring specific training in crisis de-escalation and proper restraint methods, along with detailed incident reporting as per facility policy and regulatory guidelines.

Qualifications:

  • High School Diploma with 3years' experiencein the Mental Health, Behavioral Health or Drug and Alcohol field orAssociatesdegree with 2 or moreyears' experiencein the Mental Health, Behavioral Health or Drug and Alcohol Field orHigh School Diploma/Associatesdegree and 1 year experience at ProjectTransition/ PCS Mental Health.
  • Mustpossessavalid, non-commercial driver's licensewith aclean driving recordandhave the ability tooperatecompany vehiclesin accordance withsafety regulations
  • Excellent communication,interpersonal, organizational,and time management skills.
  • Demonstrated ability to work as a part of a multi-disciplinary team.
  • High energy individual with strong workethicsand ability to multi-task.
  • Must be able to work in a self-directed manner.
  • Ability tomaintainconfidentiality.
