Case Manager Position

Utah Case Management is looking to hire individuals who have experience as a Case Manager who have worked with low income, homeless individuals and justice involved individuals. The Case Manager assists with housing related services for the client through Utah Medicaid’s Target Adult Medicaid and Housing Related Service and Supports program. We offer a flexible schedule allowing you to govern your own hours. This is a hybrid position with remote documentation and in person client visits. We are looking for dedicated employees to assist with coordinating services for our clients. We are looking for Case Managers in the Salt Lake County, UT area. This job requires going into Correctional Facilities.

Responsibilities:

• Managing clients services- weekly face-to-face contact with clients, assessing clients needs, coordinating housing services for clients, etc.
• Documentation- completing, updating, submitting, and compiling necessary documentation for clients.
• Regularly meeting and communicating with clients at their facility
• Communicating with other case managers to coordinate clients’ services.
• Work with clients to find and maintain housing
• Build and maintain relationships with clients and community partners
• Regularly attend staff meetings and company trainings
• Able to use your own devices
• Able to travel to visit clients in Salt Lake County, UT

Qualifications:

• Utah Resident and residing in Salt Lake County
• Current Case Management Certification or SSW License in the State of Utah
• Good at building and maintaining relationships with clients, clients families, and facilities
• Detail oriented
• Self motivated
• Independent and team worker
• Good communication and organizational skills

Pay Rate:

• Billable rate up to $23 (billable rate is anything that can be billed to Medicaid for the client)
• Non-billable rate $15 (non-billable is related to the position, but not client related, i.e. training, team meetings)
