Full Time Emergency Shelter Case Manager
PURPOSE: To plan, organize and implement social work intervention for homeless clients and their families in order to resolve special problems facing them and to improve their ability to transition back into society.
ESSENTIAL FUNCTIONS-JOB DUTIES: The following duties are considered essential to the job. Qualified individuals must have the ability to perform the following duties.
Perform an intake evaluation including a social history on all City Mission clients.
Establish an individualized housing and supportive service plan for each client (Client Treatment/Service Plan).
Assist clients and families in locating and utilizing community resources.
Maintain relationships with a wide variety of community agencies and facilitate communication between them and clients, their families and clinical personnel. This would include assisting clients and their families in accessing benefits such as Medicare, Medical Assistance, Veteran’s Benefits, OVR benefits, Public Assistance, and others.
Establish and maintain case files for all clients according to guidelines established by City Mission.
Evaluate social, emotional, financial, and environmental needs of clients. Counsel, advise, and support clients and their families.
Evaluate progress of every individual every 7 days (while in the shelter), to determine progress towards initial goals set. Based upon this evaluation, provide input to the Director of Shelter Services.
Coordinate statistical research, compile reports, attend scheduled case management team meetings, and facilitate staff training as directed by the Director of Shelter Services.
Perform an exit interview with clients as well as a client satisfaction survey on all clients leaving City Mission programs. For clients who have left the emergency shelters, provide a follow-up at 6 months.
SECONDARY FUNCTIONS-JOB DUTIES: The following duties are considered secondary to the essential functions listed above:
Document social histories as needed.
Intervene in emergency situations
Present case material and contribute to discussions in staff meetings.
Participate in community-related functions as a Social Service representative of City Mission.
Assist in resolving social disruptive, client-oriented situations when requested, including child abuse, adoption, custody issues, law-enforcement cases, Emergency Medicine and Family Health Service cases.
Actively participate in annual fundraising events for the agency.
Job Requirements
EDUCATION/KNOWLEDGE REQUIREMENTS:
Specific Education, Certification, And/or Licensure
Bachelor of Social Work (BSW) or related field. Preferred MSW or related field. Or an Associate's degree in Human Service field with 3 years' experience.

Essential Job-Related Experience
Two (2) year of social work experience
Preferred Job-Related Experience
Two (2) year of case management work with homeless population
SUPERVISORY RESPONSIBILITY: None
WORKING CONDITIONS: Position involves possible exposure to the following harmful elements.

This is a full-time position 40 hours per week with occasional nights and weekends.
