Position Title: Case Manager

Location: Hybrid – Field-based within assigned parishes with some remote and office work
Salary: $48,000 annually

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with flexibility for evenings and weekends as needed to meet family schedules
About The Family Village
The Family Village provides essential support and resources to children in foster care, at risk of entering foster care, or recovering from trauma such as natural disasters, domestic violence, or the loss of a parent. We meet each child’s unique needs through comprehensive case management and our boutique-style clothing closet, offering everything from beds and bedding to formula and diapers. Guided by the principle that no child in need is ever turned away, we partner with nonprofits, DCFS, first responders, and local community members to create a strong network of care. Position Overview
The Case Manager delivers trauma-informed, family-centered services to children and families as part of the My Community Cares (MCC) model. Each Case Manager carries an average caseload of 10 to 20 families, providing short-term intensive support for an average of 3 to 6 months. This role balances individualized case management with community engagement, ensuring families are stabilized, connected to supports, and equipped for sustainability post-closure. Case Managers work primarily within assigned parishes to minimize travel.
Core Responsibilities
Case Management
- Conduct family intakes and needs assessments, developing measurable and customized family support plans.
- Provide intensive case management with a minimum of 2 in-person visits per month and weekly touchpoints, flexing as needed.
- Deliver services using trauma-informed practices and the Strengthening Families Framework.
- Provide same-day concrete supports when needed, up to a $200 per-family discretionary amount before director approval.
- Assist families in overcoming barriers such as transportation, digital access, or resource navigation.
Community Engagement
- Dedicate approximately 20 percent of time to MCC neighborhood and parish-level outreach.
- Facilitate warm handoffs and referrals to community-based supports.
- Partner with DCFS, courts, schools, and local organizations to identify gaps and advocate for community-driven solutions.
- Lead and participate in MCC meetings to support collaborative, community-led strategies.
Collaboration and Reporting
- Collaborate closely with TFV colleagues, including Connect Coordinators, Parent Educators, and Concrete Services Coordinators.
- Document all case activities in the TFV case management system.
- Track service activities, demographics, outcomes, and family progress for continuous program improvement.
- Provide regular progress summaries and sustainability screenings throughout case plans and post-closure.
Skills and Qualifications
- Bachelor’s degree in social work, human services, or related field required; Master’s degree in social work preferred.
- Experience in case management, child welfare, kinship care, or community-based services preferred.
- Strong understanding of trauma-informed practices, protective factors, and family engagement.
- Effective communicator with strong advocacy and relationship-building skills.
- Organized, adaptable, and able to manage time independently in the field.
- Reliable transportation and ability to travel within assigned parishes.
Accountability and Metrics
- Average caseload of 10–20 families managed with timely service delivery.
- Completion of intakes, assessments, and individualized support plans.
- Case documentation accurate and up to date in the system.
- Referrals and warm handoffs completed with follow-up.
- Concrete supports provided within program guidelines and documented.
- Demonstrated family progress on safe parenting competencies, protective factors, and sustainability screenings.
Professional Development

Time Expectations and Travel
- Case managers are expected to meet clients where they are at. It could be at home, at their job, or a place in the community.
- Case managers are expected to meet with clients when they are able to be seen. This might be during the day, an evening, or the occasional weekend.
- We encourage case managers to flex time if they work outside of regular hours and need to offset the extra hours worked. This could look like working until 6:00PM Thursday and starting your day later on Friday.
- We do not offer mileage reimbursement but we do provide a $250 travel stipend monthly.
- Case managers are encouraged to schedule appointments in the same area on the same day to decrease how far they have to travel to see all clients.
