Case Manager

Program: Direct Services
Reports To: Clinical Director / Clinical Supervisor
Employment Status: 1099 Contractor

Position Summary

The Case Manager provides community-based support and care coordination services to children, adolescents, and families receiving behavioral health services. This role focuses on helping individuals build independence, access resources, and improve overall functioning within their home, school, and community environments.

Working under the supervision of an independently licensed clinician, the Case Manager delivers services aligned with Community Psychiatric Supportive Treatment (CPST) and Therapeutic Behavioral Services (TBS) to support stability, recovery, and successful community integration.

Essential Duties & Responsibilities

  • Provide CPST and TBS services in home, school, and community-based settings
  • Conduct ongoing assessments to identify client needs, strengths, and barriers to care
  • Assist clients in developing daily living, social, and coping skills to improve functioning
  • Coordinate care and connect clients to community resources, supports, and services
  • Support implementation of individualized service and treatment plans
  • Provide advocacy, outreach, and support to promote client engagement and retention in services
  • Assist with crisis prevention, intervention, and stabilization as needed
  • Collaborate with families, schools, courts, and other service providers to ensure coordinated care
  • Support clients in achieving educational, vocational, and personal goals
  • Provide psychoeducation related to mental health, symptom management, and life skills
  • Maintain accurate, timely, and compliant documentation, including progress notes and service records
  • Meet productivity and service delivery expectations established by the agency
  • Participate in supervision, team meetings, and ongoing training

Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, or related field preferred
  • High school diploma/GED with at least 3 years of relevant experience may be considered
  • Experience working with children, adolescents, or individuals with behavioral health needs preferred
  • Knowledge of CPST, TBS, and/or PSR services preferred
  • Understanding of trauma-informed care and community-based service delivery
  • Valid Ohio driver’s license, reliable transportation, and proof of insurance required
  • Ability to pass background check and meet all credentialing requirements

Work Environment

  • Community-based settings including homes, schools, shelters, and other partner locations
  • Requires local travel to provide services

Job Type: Full-time

Pay: From $26.00 per hour
