Case Manager

Organization: Mobile Integrative Navigation Team (MINT)
Location: Grants Pass, OR
Schedule: Full-Time, Weekdays
Pay: $20–$24/hour (DOE)

About MINT

The Mobile Integrative Navigation Team (MINT) is a grassroots, community-based organization dedicated to serving unhoused and marginalized individuals in Josephine County. We meet people where they are, literally and figuratively, providing low-barrier access to services including medical care, case management, meals, and connection to housing and treatment.

Our work is rooted in harm reduction, trauma-informed care, and dignity-first relationships. We believe trust is the foundation for meaningful change.

Position Summary

The Case Manager supports individuals accessing MINT services by helping them navigate systems and move toward stability, including housing, healthcare, and treatment. This position includes both on-site work at the Navigation Center and field-based outreach, engaging individuals experiencing homelessness where they are located.

This role is relationship-driven and requires compassion, flexibility, and the ability to work in dynamic environments.

Key Responsibilities

Client Support & Case Management

  • Conduct intakes, needs assessments, and service planning
  • Provide ongoing case management and goal tracking
  • Assist clients with obtaining identification, benefits, and documentation
  • Support applications for housing, treatment programs, and other services
  • Coordinate care with partner agencies and providers

Outreach & Engagement

  • Participate in street and community outreach efforts
  • Engage individuals experiencing homelessness in a respectful, nonjudgmental manner
  • Provide resources, referrals, and basic needs support during outreach
  • Build trust and maintain consistent follow-up with clients

Care Coordination

  • Collaborate with medical staff, peer support, and community partners
  • Facilitate referrals to mental health, substance use treatment, and primary care
  • Advocate for clients and help reduce barriers to services

Documentation & Compliance

  • Maintain accurate and timely client records
  • Complete required documentation in accordance with organizational and funding requirements

Required Qualifications

  • Experience working with unhoused, vulnerable, or high-need populations
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Valid driver’s license and ability to travel locally for outreach

Preferred Qualifications

  • Background in social services, case management, peer support, or healthcare
  • Knowledge of local resources and service systems
  • Experience with trauma-informed care and harm reduction
  • Lived experience with homelessness, recovery, or navigating systems (strongly encouraged)
