Position Overview:
The Case Manager is responsible for direct service provision to residents which include individual, group and financial counseling, client advocacy, referrals and service follow-up. In addition, the Case Manager will make the necessary linkages to assist residents in areas of permanent housing, aftercare and educational services, public assistance, Medicaid or other qualifying benefits.
Responsibilities:
- Conduct intake interviews, as assigned, to assess and evaluate residents for admission using DHS CARES system.
- Engage residents in initial and ongoing treatment processes, through individual and group counseling.
- Conduct independent living plans (ILP) on at least a biweekly basis.
- Monitor case plans for effectiveness and individual client needs.
- Assist residents in the process of locating and securing permanent housing, in conjunction with Housing Specialist.
- Assists all residents in the process of referral and placement in community based resources.
- Assess residents’ vocational and educational needs, aptitudes and interests through interviews and testing.
- Develop and maintain placement and follow-up as appropriate after assessment and counseling.
Requirements:
- Associate's Degree required.
- Two (2) or more years of current Case management experience within a residential shelter setting.
- Two (2) years of experience providing 1 to 1 counseling preferred.
- Experience with assisting eligible clients through the process of obtaining Public Assistance, Medicaid and other benefits.
- Demonstrated experience with one on one and group facilitation.
