Case Manager

The Bridge, Inc. - Alabama only – multiple locations

As a Case Manager, YOU will provide case management services to adolescents and their families in our Baldwin County Transitions program. You would serve the Foley area at various locations in the community, schools, and homes under the supervision of the program manager. This is a full-time, Monday-Friday, non-exempt position: great benefits, eligible mileage reimbursement, and a cell phone stipend.

Requirements

• High school diploma or GED, Bachelor’s degree preferred
• Valid Alabama driver’s license and acceptable driving record with proof of personal vehicle insurance
• Have access to personal transportation to meet with clients

Job Responsibilities / Essential Functions

• Provide case management services, including group and individual services for Basic Living Skills, Prevention, Parent Education, Family Support, Crisis Intervention, and other services
• Willing to work with families and clients during non-traditional hours in a variety of settings, including office, school, client’s home, or other approved community settings
• Assist in counselor/staff consultations on appropriate interventions with clients and assist in adjustments to constructive responses to program services
• Work collaboratively with agencies and maintain a professional working relationship between the Center and other agencies
• Perform basic clerical activities
• Document activities and incidents
• Participate in staff meetings, training, and in-service meetings

Pay: $10.65 - $11.86 per hour
