Our providers are trained in a variety of evidence-based techniques and therapies to help our consumers achieve optimal mental health.
The Service Facilitator and Individual Skills Developer is responsible for coordinating recovery-based services for consumers who face mental health challenges. Service Facilitators develop assessments, recovery plans, make referrals for service and facilitate team meetings.
Responsibilities Include:
- Coordinate, monitor and manage care plans, services and resources regarding consumer caseloads
- Coordinate the completion and compliance of required CCS consumer documentation
- Ensure completion of CCS required certification, continuing education, and documentation of service facilitation and individual skill work
- Complete and submit billing for all billable services and all other documentation within 48 hours
- Coordinate with community agencies and other healthcare providers on the consumer’s team
- Collaborate with interdisciplinary teams to optimize consumer outcomes
- Provide a collaborative, productive workplace environment while enriching the team environment
- Attend and actively participate in program meetings and agency meetings
Position Qualifications:
- Master's degree in social work, psychology, or closely related field
- Demonstrated experience working with individuals or families on issues related to mental health, community resources usage, case management and/or advocacy
- Commitment to quality care in strength-based, trauma-informed, recovery-focused programming
- Valid driver’s license; insurance and successful completion of a driver’s background check for work related travel
- Demonstrated written and verbal communication skills with the ability to interact with consumers in a professional, empathetic, and confidential manner
- Proficient computer skills, specifically in current versions of Microsoft Office Suite/ Google Suite; and willingness to learn new skills as needed or required
- Ability to pay attention to detail and a commitment to quality and the timely completion of duties and attainment of deadlines
- Independent workers with the ability to create and maintain their own work schedule to meet consumer needs
- Demonstrated ability to work as a member of a team
In order to qualify as a billable psychosocial rehabilitation in the CCS program, a service must:
- have been determined through the assessment process to be needed by an individual CCS participant;
- involve direct service;
- address the CCS participant’s mental health and substance use disorders to maximize functioning and minimize symptoms;
- be consistent with the individual participants’ diagnosis and symptoms;
- safely and effectively match the individual’s need for support and motivational level;
- be provided in the least restrictive, most natural setting to be effective for the CCS participant;
- not be solely for the convenience of the individual CCS participant, family or provider;
- be of proven value and usefulness; and
- be the most economic option consistent with the CCS participant’s needs.
Examples of billable services include, but, are not limited to:
- Direct Consumer Contact
- Authorized Psychosocial rehabilitation services
- Time spent at Recovery Team Meetings
- Travel Time (as long as services were rendered to the consumer)
- Documentation Time for services rendered
Pay: $27.00 - $30.00 per hour
