Job Title: Case Manager
Program: Beyond Addiction
Reports To: Clinical Supervisor / Program Director
Position Summary:
The Case Manager at Beyond Addiction is responsible for coordinating comprehensive care for women in a residential substance use treatment setting. This role requires a strong understanding of substance use disorders, recovery principles, trauma-informed care, and family systems. The Case Manager serves as a liaison between clients and external agencies to ensure continuity of care, access to resources, and successful transition planning.
Key Responsibilities:
- Conduct comprehensive assessments of client needs, including substance use history, mental health, medical, housing, legal, and social service needs.
- Develop, implement, and regularly update individualized service plans in collaboration with the client and treatment team.
- Provide ongoing case management services, including advocacy, referrals, and resource coordination.
- Maintain consistent communication and collaboration with outside agencies, including:
- Child protective services (CYS)
- Courts and probation/parole
- Medical and behavioral health providers
- Housing agencies
- Educational and vocational programs
- Coordinate and schedule appointments for clients, including medical, dental, mental health, and social services.
- Monitor client progress and compliance with treatment goals and external requirements.
- Assist clients with obtaining benefits such as Medicaid, SNAP, WIC, SSI/SSDI, and other entitlements.
- Facilitate discharge planning, including housing, outpatient treatment, employment, and childcare arrangements.
- Participate in multidisciplinary team meetings and case consultations.
- Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR).
- Ensure all releases of information (ROI) are properly completed prior to communication with outside entities.
- Provide crisis intervention and support as needed.
Qualifications:
- Minimum of 2 years of experience working in substance use disorder treatment or related field.
- Knowledge of substance use disorders, recovery processes, relapse prevention, and trauma-informed care.
- Experience working with women and families preferred.
- Strong understanding of community resources and external service systems.
- Experience coordinating care with multiple outside agencies.
- Excellent organizational, communication, and documentation skills.
- Ability to manage multiple cases and prioritize effectively.
- Proficiency in EMR systems (e.g., KIPU or similar) preferred.

Working Conditions:
- Residential treatment setting for women
- May require occasional evening or weekend hours
- Ability to respond to crisis situations in a calm and professional manner
Physical Requirements:
- Ability to sit, stand, and walk for extended periods
- Ability to lift up to 20 pounds if needed
- Ability to safely navigate a residential facility environment
Additional Requirements:
- Valid driver’s license and reliable transportation
- CPR/First Aid certification (or willingness to obtain)
