Case Manager

Job Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. Duties

• Conduct thorough assessments of clients’ needs, including mental health, addiction recovery, child welfare, vocational and housing stability
• Develop personalized care plans that incorporate crisis management, addiction counseling, and workforce development strategies
• Provide crisis intervention and behavioral health support during urgent situations to ensure client safety and stability
• Coordinate with employers, human resource departments and employee assistance programs to assist with clients' full participation in treatment while on leave
• Offer motivational interviewing techniques to encourage positive behavioral change and goal setting
• Facilitate psychoeducational and skill-based treatment groups
• Supervise and mentor junior staff or interns, fostering professional growth within the team
• Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with agency standards

Skills

• Strong expertise in crisis intervention and crisis management techniques
• Proven experience in addiction treatment and behavioral health support
• Knowledge of social work principles, child welfare systems, and insurance regulations
• Excellent communication skills with the ability to motivate and empower clients through motivational interviewing
• Ability to handle sensitive situations with empathy, professionalism, and transparency

Pay: $50,000.00 - $58,000.00 per year
