Case Manager

Treatment Specialist working with teenage population 12-17 in a residential setting

Case Manager/Rehab

Daily Duties:

  • Meet with each present client to gauge program participation and general mood and address any needs, behavioral concerns, instabilities, and refer as needed. Reflect on the current situation and set goals for the day. Record in EHR
  • Preview all daily shift notes and Incident reports since the last shift.
  • Tend to immediate daily need and or pressing issues regarding clients.
  • Provide/Assist with supervision of clientele.
  • Schedule and/or Attend sick/follow up/specialty physician visits as needed, and oversee the appointment, documentation and EHR input.
  • Input/Update following all medical appointments that all medications are retrieved from pharmacy and inputted/removed into electronic record in accordance with medication lists biweekly
  • Conduct daily medication checks to ensure medications are distributed and signed for.
  • Ensure and support youth to attend Day Program and psychotherapy as assigned
  • Provide assist with any transportation needs of the youth
  • Oversee study hour/homework.
  • Meet with every youth, every day and address their behaviors, daily function impairments, successes and failures, high risk behaviors, recent incidents etc.

Other Duties/Expectations

  • Review all client history, need for placement, and historical information to best understand their needs for behavior modification/management, medical, and therapeutic care.
  • Initial Safety Plan .
  • Ensure medi-cal cards/#’s are received and promptly transitioned to cencal.
  • Schedule intake medical/dental appts
  • Enroll youth in school at intake.
  • Initiate/Update AWOL plans as needed or identified in the CFT
  • Review intake paperwork with new youth within 24 hours of intake (unless placed on a Saturday.
  • Ensure all intake documents are signed and in electronic record. Ensure STRTP agreements are complete and sent back to SW
  • Develop/Update Needs and Service Plans monthly to align with CFT’s
  • Conduct Staying Healthy Assessments within 48 hours of intake
  • Complete activity/physical activity schedule
  • Monitor/Supervise Day Program youth and assist with homework
  • Supervise/assist study hour, assess needs for additional tutoring, ensure proper materials are in place
  • Schedule sick/follow up/specialty physician visits as needed, and oversee the appointment, documentation and EHR input.
  • Input/Update following appointments that all medications are inputted/removed into electronic record in accordance with medication lists biweekly
  • Attend Dr Sal appointments as scheduled to ensure medication documentation is complete, medications are gained and inputted, ensure all coordinating documentation is gained and filed in all locations.
  • Conduct weekly medication audits ensuring MARS, JV220’s, Med lists all sync.
  • Schedule and supervise/monitor and document family visits in person/zoom
  • Maintain resident roster/LIC 9020
  • Make CADA referrals/ schedule intake appointment as specified by SUD specialist
  • Ensure referrals to outside services IE; TBS, nutritionists, eating d/o clinics, LGBTQ groups are complete and services implemented as needed in collaboration with the clinical Tx team.
  • Arrange, facilitate and conduct CFT”S and ensure updating of white boards for new privileges for all CFT’s.
  • Request/Attend IEP’s and 504’s for all students as needed.
  • Coordinate regularly with school admin, liaisons, teachers, and counselors at the youth’s school to address behavior, academic, attendance needs are addressed.
  • Update Google Calendar with upcoming CFT and all appts for each youth
  • Ensure Treatment binders and in house client binders stay up to date
  • Input all services in identified Hipaa compliant program ie; SmartCare/ Extended Reach
  • Complete daily notes for each shift/client.
  • Review weekly point sheets with the youth during weekly sessions so they have an idea of expectation and where they are regarding functional skills. Remind of % goal on N&S plan
  • Review incident reports daily and assist youth in reflecting on their behaviors following major incidents.
  • Crisis Intervention/ Prevention with youth
  • Schedule and implement weekly motivational/psycho-education group
  • Initiate and develop safety plans upon intake

Job Type: Full-time

Pay: $26.00 - $30.00 per hour
