Case Manager – Residential Treatment Center (RTC)

Position Overview

Guiding Light Haven is seeking a qualified and experienced Case Manager to support youth in a licensed Residential Treatment Center (RTC) operating in accordance with Texas Minimum Standards under HHSC.

The Case Manager is responsible for coordinating individualized service planning, monitoring client progress, maintaining compliance documentation, and ensuring collaboration with families, placing agencies, and interdisciplinary treatment teams.

This position requires a minimum of one (1) year of case management experience, preferably in a residential treatment, foster care, behavioral health, or child welfare setting.

Minimum Qualifications

  • Minimum 1 year documented case management experience
  • Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field (preferred)
  • Knowledge of Texas HHSC RTC/GRO Minimum Standards
  • Experience working with at-risk youth, trauma-informed care, or behavioral health populations
  • Strong documentation and case file management skills
  • Ability to pass background checks required by HHSC
  • Valid Texas driver’s license

Essential Duties & Responsibilities

Service Planning & Coordination

  • Develop and implement individualized service plans in collaboration with clinical staff
  • Participate in treatment team meetings and case reviews
  • Coordinate services with placing agencies, CPS, probation, schools, and healthcare providers
  • Ensure goals are measurable, trauma-informed, and aligned with regulatory standards

Documentation & Compliance

  • Maintain complete and accurate case files in compliance with Texas Minimum Standards
  • Ensure required documentation is completed within required timeframes
  • Monitor incident reports and ensure follow-up documentation
  • Assist in audit preparation and licensing reviews

Client Advocacy & Monitoring

  • Monitor resident progress toward service plan goals
  • Conduct regular check-ins with residents
  • Support discharge planning and aftercare coordination
  • Ensure family engagement when appropriate

Interdisciplinary Collaboration

  • Work closely with therapists, direct care staff, administrators, and medical providers
  • Communicate changes in resident status to appropriate parties
  • Participate in staff meetings and required trainings

Knowledge & Skills

  • Understanding of trauma-informed care principles
  • Familiarity with child welfare systems in Texas
  • Ability to manage multiple cases simultaneously
  • Strong organizational and communication skills
  • Professional boundaries and ethical practice

Compliance Requirements

This position must comply with all applicable standards established by the Texas Health and Human Services Commission, including:

  • Background checks
  • Training requirements
  • Documentation standards
  • Confidentiality laws (HIPAA where applicable)
  • Mandatory reporting requirements

Preferred Qualifications

  • Experience in RTC or GRO setting
  • Knowledge of DFPS/CPS processes
  • CPR/First Aid Certification
  • Crisis intervention training
