DEFINITION:
The Case Manager ensures that youth records are kept up to date, current and discharged; maintains a comprehensive filing system and computer database for document retention. Acts as a liaison with Division of Children’s Services or legal guardian to plan for assigned services (adoption, foster care, parent training, substitute care, family centered services, permanency planning, family and individual or group therapy, and protective and/or in-home services) that require specialized knowledge to ensure proper care and support for youth. Some travel is expected for this position. Occasional youth transport.
EXAMPLES OF JOB RESPONSIBILITIES:
· Collect and maintain information needed to ensure youth electronic and paper charts are up to date.
· Research and identify community resources and services prior to processing youth for return to family.
· Perform clerical duties to generate documents or important memos, as necessary.
· Work directly with clinical team, Children’s Division, and additional referral sources to ensure compliance with state licensure.
· Cooperate with state agencies and law enforcement agencies in matters relating to assigned youth.
· Should be able to work independently, identify risks, assess needs, and develop intervention strategies. Should be able to interpret and apply regulations, agency policies and procedures that are associated with areas of assignment.
· Send intake packets to Children’s Division workers. Assist treatment team in the intake process and decision making. *Ensure that information on new youth is properly distributed to necessary administrative staff.*
· Evaluate family visiting applications with Children’s Division and determine whether to approve or deny the individual’s request to visit with the youth. Set up passes for the youth and ensure that anyone wishing to come on-site is preapproved and on the youth’s contact list. Case managers will supervise on-site visits during the week, if applicable.
· Assist with making phone calls (one) call per week for 15 minutes, for every youth. Document all calls made to the Children’s Division or other important figures on communication logs.
· Put court dates and scheduled visitations, FST’s, etc., on the master calendar.
· Organize current youth files and archive files.
· Prepare monthly reports.
· Receive, review, and file incoming youth’s paperwork.
· Maintain youth’s school records (retrieved from on-site school administrator). The youth’s file should have the school’s information packet, a copy of their birth certificate, and a shot record. If one or both is missing, contact the youth’s Children’s Division Case Worker. A copy of the service placement agreement should also be included with the school paperwork.

QUALIFICATIONS AND REQUIREMENTS:
A Bachelor’s degree or above in Social Work, Criminal Justice, Education, or another related field is required. Must be able to pass a background check, have a driver’s license, and current auto insurance.
Strong organizational skills, public relation skills, thoroughness and technical capacity are preferred skills. The employee is frequently required to stand, walk short distances between buildings, and transport youth for appointments or clothing vouchers. May be required to use PCI if a youth becomes dysregulated while in your care.

Pay: $16.00 - $20.00 per hour
