This is a hands-on, community-based role where you’ll build strong relationships, coordinate meaningful services, and help families move toward greater stability and well-being.

Position Summary: The Case Manager provides comprehensive, strength-based support to families we work alongside at the Family Resource Center. This position supports families in achieving stability and greater stability and well-being by assessing needs, coordinating services, and collaborating with community providers, including the New Hampshire Division for Children, Youth and Families (DCYF). The Case Manager is responsible for maintaining compliance with all applicable New Hampshire regulations, agency policies, and documentation standards.

Essential Duties and Responsibilities:

  • Maintain and manage an assigned caseload of families in accordance with program expectations
  • Conduct comprehensive assessments to identify family strengths, needs, and service goals
  • Partner with families to develop, implement, and monitor individualized service/treatment plans
  • Provide weekly face-to-face contact with families to support progress toward identified goals and support families in navigating complex needs
  • Collaborate regularly with DCYF and other community-based organizations
  • Coordinate and facilitate transportation to services and resources within the community to support engagement
  • Maintain accurate, timely, and compliant documentation, including progress notes, service plans, and required reports
  • Participate in case reviews, supervision, team meetings, and ongoing professional development
  • Provide advocacy on behalf of families to ensure access to appropriate services
  • Ensure adherence to confidentiality standards and all applicable federal and New Hampshire state regulations

Minimum Qualifications: Education and Experience:

  • Bachelor's degree in psychology, Social Work, Human Services, or a closely related field and a minimum of two (2) years of experience working with children and families

Additional Requirements:

  • Valid driver’s license and access to reliable transportation
  • Ability to travel throughout assigned service areas within New Hampshire
  • Successful completion of all required criminal background checks.
  • Strong interpersonal, organizational, and communication skills
  • The schedule may include evening hours to accommodate family availability.

Pay: $18.00 - $24.00 per hour
