Job Purpose
Reports to the Director of Housing & Resident Services. The Case Manager serves as a point of contact for individuals experiencing homelessness after they have transitioned from intake to residence in one of the homes, and provides regular case management to support the overall well-being of current families in one home.
Responsibilities
Case management:
- Coordinate an intentional transition from the intake and welcome phase with the Programs, Outreach and Intake Manager (POIM) to case management.
- Maintains caseload of families: conducts strengths based needs assessment; develops family partnership agreements with clients
- Creates self identified measurable goals to ensure families are successful in accomplishing their identified goals.
- Serves as the main contact for current mom case management needs, providing a level of support based on each families’ needs.
- Provides support services for current moms, including ensuring homes have up-to-date information regarding community resources and locating specialized resources, transportation needs to appointments etc.
- Coordinates, under the Director of Housing and Resident Services approval, all financial requests (loans, scholarships, emergency expenses, etc.) for current moms.
- Writes letters of recommendation for moms when requested.
- Collaborates with POIM to connect with local programs and agencies to locate services for families.
- Represents Zelie’s Home in court on behalf of current and alumnae moms as needed.
- Maintains and updates all current mom files; prepares them to be archived annually.
- Inputs case notes, goals, etc. to client records in the reporting system on a regular basis.
- Maintains accurate and consistent collection, documentation and organization of case management assessments and corresponding referral information.
- Document referrals and enrollments in Zelie’s Home programming.
- Inputs entry and exit data to client records in the reporting system on a regular basis.
- Responsible for maintaining data and reporting outcomes on a monthly basis to the Director of Housing and Resident Services.
Mom Relations:
- Meets with current moms weekly, check-ins expected daily.
- Model the importance of personal presence with staff and moms by spending time in community regularly
- Coordinates ‘Mom Updates’ and the sharing of information about particular moms’ cases. Ensure coordination of moms’ counseling schedule, including the sharing of mom updates on a weekly basis.
- In conjunction with the POIM, attends Community Night Dinner and coordinates house meetings..
- Coordinates and assigns ride requests for current moms to staff and Community Life Assistants members ensuring moms feel advocated for and supported.
- With leadership from the House Manager, assist wjth the upkeep of the house; make sure the “First Impressions Maintenance” is being done in community / tour-ready space.
- In conjunction with the POIM, coordinates support for fathers of the babies served.
- Maintains appropriate professional boundaries and adheres to ethical standards in all interactions.
Collaborative Efforts
- Serves in the on-call rotation for in-home emergencies.
- Participate in weekly and biweekly meetings with appropriate teams for easy and transparent information sharing.
- Consult the leadership team at home prior to asking a mom to leave. If issues arise and/or final decision is needed, consult the Director of Housing and Resident Services
- Be available for emergency/crisis/major issues that arise at the house. Provide onsite support to the CLAs during crisis or emergency situations. Be intentionally aware of conflicts, difficulties or stressors. Coordinate with a mental health provider to deescalate the situation. Follow up with the Director of Housing and Resident Services.
- Mediates and leads important conversation/safe space conversations with moms, Community Life Assistants, staff, volunteers etc as needed.
- In conjunction with the Social Worker and in alignment with the evidence-based programming, the Case
Manager coordinates weekly and monthly schedules for current moms.
- Provides transportation for moms and their children on as needed basis with company vehicles.
- Assumes any additional projects or tasks as assigned by the Director of Housing and Resident Services.

Qualifications/Skills
- BA in a related field (i.e. specialization in social, behavioral or human services).
- At least 2 years of case management experience
- Valid driver’s license with good driving record and proof of insurance.
- Computer literacy in Google Suite, Office Suite, and Slack.
- Knowledge of resources in the Cleveland community
- Strong interpersonal skills and engagement skills.
- Ability to work effectively as part of a team.
- Demonstrates experience and competency in working in a culturally diverse setting, and an understanding of the intersectional nature of homelessness, poverty, abuse, disability, substance abuse and mental illness.
- Excellent organizational, supervisory, and communication skills, with proficiency in engaging diverse audiences.
- Experience working with vulnerable populations, addiction and poverty.
- Experience with crisis intervention and/or mediation.
- Alignment with Zelie’s Home core values and sanctity of human life.
- Must be able to lift up to 25 pounds occasionally (e.g., moving donations, baby supplies, or household items).
- Ability to climb and descend stairs multiple times per day, as the home includes multiple floors.
- Comfortable working in both office and residential settings, with frequent movement between rooms and interaction with families
Preferred Qualifications
- Certifications in trauma-informed care, motivational interviewing.
- LSW, LISW
Job Type: Full-time
Pay: $18.00 - $22.00 per hour
