Case Manager

Overview:

We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a strong background in social work, behavioral health, crisis intervention, workforce development, motivational interviewing, addiction counseling, and community resources.

Responsibilities:

- Conduct comprehensive assessments of clients' needs

- Develop individualized care plans and goals

- Provide support services to clients

- Coordinate referrals to community resources and services

- Monitor and evaluate clients' progress towards goals

- Collaborate with interdisciplinary teams to ensure holistic care

- Maintain accurate and up-to-date client records

Experience:

- Master's level interns encouraged to apply and can do clinical hours during employment

- Bachelor's degree in Social Work or related field

- Proven experience in social work, behavioral health, crisis intervention, workforce development, Medicare, motivational interviewing, addiction counseling, and community resources

- Strong communication and interpersonal skills

- Ability to work effectively in a fast-paced environment
