Job Description
Matthew 25 is a Nashville, TN based non-profit that serves homeless men, including veterans, those out of incarceration, and other men experiencing homelessness. Matthew 25’s mission is to offer Housing, Help and Hope. We offer Help in facing the issues causing homelessness. We offer Hope by walking alongside each of our clients, encouraging them in their recovery. Finally, we offer Housing for those committed to break out of homelessness, as they focus on themselves and their goals. Transitional housing residents participate in weekly case management and supportive meetings, with goals to obtain income and independent housing.
Position Summary: This position works primarily with our Veteran residents. The case manager will hold a caseload (max of 20), meeting with them weekly while they work towards individualized goals that will assist them in obtaining stable housing.
The Case Manager will be expected to work on-site the majority of the full-time work week, as this position is not remote work.
Primary Duties:
- Establish Individualized Service Plans at intake, and revise goals every 90 days based on the individual's needs at that time.
- Schedule and keep weekly appointments with clients to assess progress towards goals. Must have enhanced time-management skills.
- Timely documentation with all records for clients. Maintain upkeep of client documentation obtained while client is in our program.
- Communication with all staff regarding the care of each client, reaching out to VA staff when extra support or information is necessary.
- Connecting with community partners in the CoC, VA, employment support organizations, recovery-based organizations, and landlords/property managers.
- Utilize community-based resources to assist residents with meeting goals. Follow up with resources on client’s behalf when needed.
- Facilitate a life or recovery skills group to the residents either weekly or bi-weekly.
- Report critical incidents immediately to the leadership team.
- Must be willing to transport residents in the Matthew 25 van when needed.
- Participate in Matthew 25 team meetings, including VA liaison meetings, and outside community collaboration meetings.
- Maintain strict confidentiality of all client information
- Assist Matthew 25 staff with other duties as needed.
- Uphold the mission of Matthew 25 by holding each resident to the program policies, as well as showing each resident respect and the love and grace of God.
Qualifications:
- Candidates should hold a master’s degree or a bachelor’s degree with relevant experience in Social Work, Mental Health, Psychology, or another Human Services field.
- Valid Driver’s license.
- Job requires lifting up to 25lbs, sitting, walking, and standing.
- Must pass a background check and a drug screen.
Preferences:
- At least 2 years of experience working in a human services or related field.
- Knowledge or experience with the Homeless Management Information System (HMIS), VA Grant Per Diem (GPD) programs and Nashville-based community resources.
- Motivational Interviewing, and/or working with individuals experiencing homelessness, substance use, and/or mental illnesses.
- Lived experience as a Veteran. Lived experience of a housing crisis.
- Possess proficient computer skills in Outlook, Word, Excel, PowerPoint, and Microsoft Teams.
