Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. This role is essential in providing support and guidance to individuals facing various challenges, including mental health issues, addiction, and social barriers. The Case Manager will work collaboratively with clients to develop personalized care plans, facilitate access to resources, and ensure the delivery of effective services that promote well-being and recovery.
Duties
- Conduct comprehensive assessments to identify client needs and develop individualized service plans.
- Provide crisis intervention and management to clients in urgent situations.
- Offer addiction counseling and support for individuals struggling with substance use disorders.
- Collaborate with healthcare providers and community organizations to coordinate care and resources.
- Utilize motivational interviewing techniques to empower clients in their recovery journey.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
- Stay informed about relevant policies, including Section 8 housing assistance, Medicare services, and child welfare regulations.
- Engage in workforce development initiatives to assist clients in gaining employment skills.
Skills
- Proficient in crisis intervention and crisis management strategies.
- Knowledgeable in behavioral health practices and social work principles.
- Experience with childcare services or programs is a plus.
- Strong mentoring abilities to guide less experienced staff or volunteers.
- Familiarity with addiction counseling techniques and resources.
- Excellent communication skills for effective client interaction and collaboration with multidisciplinary teams.

Pay: $22.00 - $23.00 per hour
