Case Manager

Job description:

The Case Manager provides comprehensive support and guidance to individuals transitioning from incarceration back into the community. The role involves developing individualized reentry plans, connecting clients with essential resources, and assisting them in overcoming barriers related to housing, employment, mental health, substance use, and legal obligations. The goal is to promote successful reintegration, reduce recidivism, and support clients' personal growth and stability.

Key Responsibilities:

  • Conduct thorough intakes and assessments to identify clients’ needs and strengths.
  • Develop personalized reentry plans in collaboration with clients, focusing on areas such as housing, employment, healthcare, substance use treatment, and legal compliance.
  • Connect clients to community resources, including housing services, job training programs, mental health and substance abuse treatment, legal assistance, and social services.
  • Provide ongoing case management, monitoring client progress, and adjusting plans as needed.
  • Offer advocacy and support to clients navigating complex systems and overcoming barriers.
  • Maintain detailed case notes, documentation, and progress reports in compliance with organizational policies.
  • Collaborate with community partners, service providers, courts, parole/probation officers, and other stakeholders to coordinate care and services.
  • Offer motivational interviewing and other evidence-based approaches to encourage positive behavioral change.
  • Promote client accountability and adherence to legal conditions, while providing encouragement and support.
  • Stay informed about relevant policies, laws, and community resources related to reentry.
  • Attend team meetings, trainings, and professional development activities.

Qualifications:

  • Experience working with justice-involved populations or in reentry or social services.
  • Knowledge of the criminal justice system, community resources, and barriers faced by reentry clients.
  • Strong communication, interpersonal, and crisis management skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Cultural competence and sensitivity to diverse populations.
  • Valid driver’s license and reliable transportation may be required.
