- Conduct comprehensive patient assessments to determine individual needs and develop tailored care plans.
- Coordinate multidisciplinary teams to ensure timely and appropriate delivery of services across hospital units such as ICU, PICU, emergency medicine, and trauma centers.
- Manage discharge planning processes by collaborating with social workers, physicians, and families to facilitate smooth transitions from hospital to home or other care facilities.
- Perform utilization review and management to ensure compliance with healthcare policies like HIPAA and optimize resource utilization.
- Utilize Electronic Health Record (EHR) systems and Electronic Medical Record (EMR) systems to document patient interactions accurately and efficiently.
- Review medical documentation including CPT coding, ICD-9/10 coding, and ICD coding to support billing and compliance efforts.
- Engage in intake processes for new patients, ensuring all necessary information is collected for effective case management.
- Support behavioral health initiatives by coordinating services for patients with mental health needs or complex social situations.
- Maintain thorough knowledge of hospital protocols, including Level I and Level II trauma center procedures, hospice care standards, and pediatric care requirements such as those in the PICU. Experience
- Proven experience in critical care environments such as ICU or emergency medicine settings.
- Strong background in hospital medicine with familiarity in acute care and managed care environments.
- Experience working within Level I or Level II trauma centers is highly desirable.
- Knowledge of medical terminology, discharge planning, utilization review, and case management best practices.
- Demonstrated ability to navigate EMR/EHR systems efficiently while maintaining HIPAA compliance.
- Prior experience with social work or behavioral health coordination is advantageous.
- Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams in high-pressure situations.
