Job Overview
The Case Manager is responsible for engaging older adults at the Bay Ridge Center and Connects locations, supporting their overall wellness, independence, and access to essential services. This role includes conducting case management activities, developing service plans, monitoring progress, and determining appropriate case closure. The Case Manager also plays a key role in connecting members to public benefits and community resources.
Key Responsibilities
- Manage assigned caseloads, develop individualized service plans, monitor client progress, and determine case closure when appropriate.
- Support older adults in maintaining wellness, independence, and quality of life through ongoing case management.
- Assist clients in applying for public benefits, including SNAP, Medicare, Medicaid, HEAP, SSI, SCRIE, and other entitlement programs.
- Maintain accurate and up-to-date case records, complete required documentation, and prepare statistical and program reports.
- Conduct data entry and outreach to engage and support clients effectively.
- Assist with membership enrollment and maintain updated member files.
- Promote cost-effective and high-quality interventions while addressing psychosocial and motivational barriers.
- Collaborate with program leadership to design and implement culturally responsive and inclusive programming that serves individuals of diverse backgrounds, including different faiths, ethnicities, genders, sexual orientations, and abilities.
- Adhere to all professional standards, organizational policies, and regulatory requirements.
Recommended Qualifications
- Bachelor’s degree in Social Work, Education, Human Services, or a related field preferred.
- Experience working with older adults strongly preferred.
- Knowledge of public benefits and human services systems is highly desirable.
- Strong organizational, communication, and interpersonal skills.
- Ability to manage multiple cases, deadlines, and reporting requirements effectively.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) and Google Workspace (Docs, Sheets, Forms, Slides).
- Experience with database systems and case management software is a plus.
Pay: $27.00 per hour
