Case Manager

Role Description

Aadhar - Mental Wellness is seeking a compassionate and organized Case Manager for a full-time, on-site position in Plano, TX. The Case Manager will be responsible for providing individualized support to clients, developing care plans, coordinating services with mental health professionals, and monitoring client progress. Additional responsibilities include advocating for client needs, maintaining detailed case documentation, and collaborating with community resources to provide comprehensive support.

Qualifications

• Experience in case management, client advocacy, and crisis intervention
• Knowledge of mental health principles, therapeutic approaches, and community-based services
• Strong organizational, time management, and multitasking skills
• Excellent written, verbal, and interpersonal communication skills
• Proficiency in maintaining accurate records, preparing reports, and upholding confidentiality
• Ability to work effectively in a diverse, team-oriented environment
• Minimum of a Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Master’s degree preferred)
• Licensure or certification in social work or a related field is an advantage
• Experience in providing services to underserved or diverse populations is highly desirable

Pay: $40,000.00 - $50,000.00 per year
