Job description:

We are seeking one experienced Case Manager to join our team in the Greenville and Spartanburg, South Carolina area - part-time.

Minimum Qualifications:

  • At least two (2) years of Case Management experience
  • Bachelor’s degree (Candidates who do not meet these requirements will not be considered.)

Position Overview:
Our agency helps individuals remain safely in their homes and avoid premature nursing home placement. Case Managers provide person-centered, coordinated care to Medicaid participants.

Key Responsibilities:

  • Develop, coordinate, monitor, and evaluate individualized care plans
  • Manage a full caseload of participants
  • Facilitate interdisciplinary team approaches
  • Monitor participant progress and adjust services as needed
  • Make appropriate referrals to community resources
  • Conduct regular in-person home visits
  • Maintain accurate and timely electronic documentation

Requirements:

  • Ability to work independently with minimal supervision
  • Strong ability to follow directions, meet deadlines, and manage a caseload effectively
  • Sound judgment and decision-making skills
  • Reliable personal automobile (travel is required)
  • Valid driver’s license and current auto insurance
  • Excellent written and verbal communication skills
  • Strong computer and documentation skills

Compensation & Schedule:

  • Full and part time positions
  • Gas expenses will be provided
  • No benefits are provided
  • Paid training is offered

QUALIFICATIONS

  • Ability to analyze complex situations and recommend appropriate courses of action.
  • Strong verbal and written communication skills. Must be able to write cohesive and comprehensive documentation.
  • Ability to handle confidential and sensitive information professionally with discretion and integrity.
  • Excellent organizational and time management skills
  • Ability to establish and maintain effective working relationships with clients, staff, & community members.
  • Ability to work independently and collaborate with others.
  • Ability to organize and conduct several tasks and projects simultaneously.
  • Ability to handle conflicts.
  • Ability to travel within a designated geographic area for in-person visits
  • Effective computer skills, including navigating multiple systems and keyboarding

Additional Requirements:

* Must pass a National Criminal Background Check

* Must have a valid driver's license and produce 10 years of driving record

* Must maintain cell phone service

* Must demonstrate skills in computer usage

* Must attend orientation and training for new Case Managers

* Must agree to accept a minimum of thirty-five (35) cases

* Must have or be willing to obtain an updated TB test

* Required education: BA/BS degree or certification

Hiring 1 candidates for this role

Training: You must complete a one week paid training class. The dates and times will be provided later.

Orientation: You must complete a non-paid orientation class. The dates and times will be provided later.

Job Type: Full-time

Salary: Full-time $33k to 42k

Pay: $35,000.00 - $42,000.00 per year
