Job Summary: The Case Manager provides trauma-informed, client-centered support and advocacy to survivors of domestic violence, sexual assault, stalking, and human trafficking. This role involves assisting clients in accessing resources, developing safety and service plans, and navigating systems, including legal, housing, healthcare, and social services.
Duties and Responsibilities:
Direct Client Services:
- Provide intake and ongoing case management services to victims and survivors.
- Conduct comprehensive needs assessments and develop individualized service plans.
- Provide crisis intervention, emotional support, and safety planning.
- Assist clients in obtaining housing, legal advocacy, medical care, employment, transportation, childcare, and other resources.
- Maintain a survivor-centered and empowerment-based approach.
- Rotating on-call schedule
Advocacy & System Navigation:
- · Support clients in navigating complex systems such as law enforcement, court, child protection, and public benefits.
- · Accompany clients to court hearings and other appointments as needed.
- · Collaborate with attorneys, healthcare providers, and social service agencies to coordinate care.
Documentation and Compliance:
- · Maintain accurate, timely, and confidential client records in accordance with the organizational policies and funding requirements.
- · Complete all reports, assessments, and documentation required for grants and other funding streams.
- · Ensure services are provided in accordance with confidentiality laws and HIPAA where applicable.
Collaboration and Outreach:
- · Participate in regular staff meetings, case reviews, and multidisciplinary team meetings.
- · Develop and maintain relationships with community partners to enhance service coordination.
- · Provide education and outreach to community members and service providers about domestic and sexual violence issues.
Organizational Support:
- · Contribute to program development and evaluation efforts.
- · Support shelter or hotline coverage if applicable.
- · Participate in staff training, professional development, and self-care practices.
- Work towards obtaining Domestic Violence and Sexual Violence Advocate Certification
Qualifications:
- · Bachelor’s degree in Social Work, Psychology, Criminal Justice, or related field (or equivalent experience).
- · Experience working with survivors of domestic and/or sexual violence preferred.
- · Strong understanding of trauma-informed and survivor-centered practices.
- · Excellent communication, organizational, and problem-solving skills.
- · Ability to work independently and as part of a team.
