Case Manager

Case Manager – Residential Treatment ProgramHouston, TX

We are seeking a dedicated and organized Case Manager to join our residential treatment team serving youth in a structured therapeutic environment. The ideal candidate will have experience working with children and adolescents involved in behavioral health, foster care, or residential services and possess strong communication, documentation, and coordination skills.

Responsibilities: Coordinate and manage client services, treatment planning, and case documentation Maintain regular communication with placing agencies, guardians, schools, and treatment providers Schedule and participate in staffing meetings, appointments, and family/team meetings Ensure all client records, assessments, and treatment plans remain compliant and up to date Assist with intake and discharge planning processes Monitor client progress and provide updates to interdisciplinary team members Coordinate educational, medical, psychiatric, and therapeutic services for residents Advocate for clients’ needs while supporting treatment goals and behavioral expectations Maintain confidentiality and professionalism in all communications and documentation Participate in on-call or crisis response duties as needed

Qualifications: Bachelor’s degree in Social Work, Psychology, Human Services, Criminal Justice, or related field preferred Experience working in residential treatment, foster care, juvenile services, behavioral health, or child welfare preferred Strong organizational and time management skills Excellent written and verbal communication skills Ability to manage multiple cases and deadlines effectively Proficient in Microsoft Office and electronic documentation systems Valid driver’s license and reliable transportation preferred Ability to pass background checks and meet state regulatory requirements

Compensation & Schedule: Full-time position Monday–Friday schedule with occasional after-hours responsibilities as needed Monthly pay schedule Competitive salary based on experience
