Role: Case Manager

Division: Adults Division

Supervisor: Division Director

Location: In Office/In the field

Requirements:

  • Must pass criminal background check
  • Must possess and maintain current NV Drivers License, automobile insurance and clean driving record.
  • At least one year experience working with adults who have developmental disabilities, physical disabilities, or mental health disabilities.
  • Intermediate use of computer applications such as Excel, Word, and other PC based programs.
  • Ability to multi-task effectively and be flexible amidst frequent changes to work flow.
  • Excellent organizational skills to appropriately manage large amounts of information.
  • High level of interpersonal skills to handle sensitive and confidential situations appropriately.
  • Commitment to a person-centered, culturally competent philosophical base.

Education:

Some college level courses in a Human Services related field from an accredited university/college. Bachelor's Degree preferred but not required.

Overview:
The Case Manager assists the QIDP and Division Director in overseeing direct support contractors in the Shared Living Provider role and coordinates support and treatment programs to ensure a person-centered plan for individuals served. To ensure all regulatory requirements are met.

Key Responsibilities:

  • Assist and support QIDP in development, understanding, and achievement of personal goals for individuals’ habilitation plans.
  • Assist and support QIDP in coordinating effective communication between individuals with disabilities, their family members, guardians, coworkers, and other professionals, adhering to all legal and ethical requirements and maintaining a high quality of care and customer service.
  • Report any home deficiencies, to include any Shared Living Provider concerns.
  • Assist and Support QIDP monitoring the effectiveness of services provided and ensure that individuals, families, guardians, the State of Nevada, and other customers meet internal quality standards.
  • Assist and Support QIDP with developing professional relationships both internally and externally to facilitate meeting the needs of clientele and achieving departmental/organizational goals.
  • Under the supervision of QIDP complete all necessary documentation and paperwork to ensure that individuals receive and maintain entitlement benefits, contracted services, community resources, and basic needs, including rep payee application.
  • Maintain appropriate boundaries with individuals receiving services and their family members, while following organizational policies and procedures to ensure the best possible care for the individuals receiving services.
  • Using Person Centered Planning assist the individual collaborate with the person to identify and develop their personal goals.
  • Support schedule and attend Individualized Service Plan (ISP) meetings and other informational meetings to ensure the person's needs are met.
  • Under the supervision of QIDP, assist with writing individuals Habilitation plans and Action plans from SLP home environmentals.
  • Assist and support the QIDP in applying for and obtaining community resources and benefits such as housing, food stamps, Medicaid, and other entitlements.
  • Under supervision of QIDP, help the person locate a suitable residence based on their needs and preferences.
  • Assist and support QIDP in accessing generic community resources and informal support that can help them achieve their goals.
  • Work with the person to identify and build natural support networks consisting of family, friends, and other associates.
  • Provide crisis management support and problem-solving assistance as needed. Report all crisis management to QIDP, and collaborate on problem solving.
  • Assist the QIDP in individual budget planning and scheduling and keeping appointments.
  • Assist and support QIDP in observing Shared Living Providers supported as provisioned by Waiver services.
  • Report to QIDP, any health and welfare concerns.
  • Complete all required paperwork as assigned on behalf of the person served accurately and in a timely manner.
  • Under supervision of QIDP, conduct home visits to observe the person's living environment to ensure their health and welfare needs are being met.
  • Assist and support QIDP to provide information to the Service Coordinator to allow for evaluation and assurance of support services provided in the ISP.

Job Type: Part-time

Pay: $18.00 - $22.00 per hour
