Case Manager

LVN/RN Case Manager
We are currently seeking a dedicated and organized LVN or RN Case Manager to join our team. This position is available as either part-time or full-time and requires in-office attendance.

Responsibilities include:

• Coordinating and managing patient care plans
• Communicating with physicians, patients, and caregivers
• Reviewing and maintaining patient documentation and charts
• Scheduling visits and ensuring timely follow-ups
• Assisting with authorizations, referrals, and care coordination
• Monitoring compliance with healthcare regulations and company policies
• Collaborating with clinical and administrative staff to ensure quality patient care

Requirements:

• Active California LVN or RN license
• Strong communication and organizational skills
• Ability to multitask in a fast-paced environment
• Previous case management or home health experience preferred
• Must be able to work in-office
• Bilingual candidates preferred

Pay: From $35.00 per hour

Work Location: In person
