Case Manager

Position Summary
The Case Manager is responsible for assisting clients in our detox and residential treatment programs by coordinating resources, addressing patient needs, and helping remove barriers to successful treatment. This role provides direct support to clients with legal, family, financial assistance, and discharge-related matters while they are in care.

The Case Manager is responsible for maintaining communication records, coordinating with outside agencies when authorized, and helping ensure clients receive the support and guidance needed during their stay in accordance with facility policies and State and Federal regulations.

The ideal candidate is organized, compassionate, professional, and able to manage multiple priorities in a fast-paced treatment environment.

Essential Duties & Responsibilities

• Assist clients with treatment-related needs and concerns during their stay
• Provide support, guidance, and advocacy throughout treatment
• Coordinate communication between clients, families, attorneys, probation officers, courts, and outside agencies as authorized
• Assist clients with legal matters such as court letters, compliance verification, required documents, and approved communications
• Help clients with benefit applications, including food stamp applications and community resources
• Maintain accurate case notes, records, and documentation
• Assist with discharge planning and aftercare resource coordination
• Work collaboratively with clinical and operations staff to support patient progress
• Ensure confidentiality and compliance with HIPAA, BBHC, DCF, and facility standards
• Assist with resolving day-to-day client concerns in a professional manner

Qualifications

• Previous case management, behavioral health, substance abuse, detox, or residential treatment experience preferred
• Familiarity with BBHC and DCF standards preferred
• Experience handling professional/legal communications preferred
• Strong organizational, communication, and documentation skills
• Ability to multitask in a fast-paced environment
• Bilingual (English/Spanish) is a plus
• Proficient computer skills

Pay: From $20.00 per hour
