Case Manager

We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. • Coordinate with multidisciplinary teams including nursing, social work, behavioral health, and physicians to ensure continuity of care across inpatient and outpatient settings.
• Conduct thorough intake assessments and facilitate discharge planning to promote safe transitions from hospital to home or other care facilities..
• Utilize electronic health record (EHR) systems and electronic medical record (EMR) systems efficiently for documentation, case tracking, and communication (Compu Care).
• Experience working with EMR/EHR systems while maintaining compliance with HIPAA regulations.
• Excellent understanding of medical terminology along with skills in discharge planning and case management processes.
• Ability to collaborate effectively with healthcare professionals across disciplines such as social work and behavioral health.
• Familiarity with managed care principles, utilization management strategies, and patient service best practices is preferred.
