Case Manager

Position Overview

The Case Manager plays a critical role in supporting individuals experiencing homelessness through our 30- and 90-day continuous stay programs. This position focuses on helping guests overcome housing barriers and achieve long-term housing stability using Housing First, harm reduction, and trauma-informed care approaches.

• Location: LCS Main Site
• Status: Full-time, Exempt
• Salary: $48,840 annually
• Reports to: Case Manager Supervisor
• Effective Date: May 15, 2026

Key Responsibilities

• Provide direct, client-centered case management services
• Support guests in securing housing, employment, and healthcare access
• Develop individualized service plans focused on stability and well-being
• Collaborate with internal teams and community partners to deliver wraparound services
• Maintain accurate, timely documentation in HMIS (Clarity or similar systems)
• Uphold trauma-informed, culturally responsive, and dignity-centered practices

Minimum Qualifications

• Associate or bachelor’s degree in social work, psychology, public administration, or related field (or equivalent experience)
• At least 2 years of experience in case management, homeless services, or similar work
• Experience with HMIS platforms and strong documentation skills
• Excellent communication, organization, and problem-solving abilities

Preferred Qualifications

• Knowledge of Housing First, HUD guidelines, and Coordinated Entry
• Experience with data reporting tools (e.g., Excel)
• Background in grant compliance, performance improvement, or training support
