POSITION: Case Manager

REPORTS TO: Program Director

REQUIREMENTS: High School Diploma/GED

Criminal Check Approval (NYS & FBI)

GENERAL RESPONSIBILITIES:

Assist residents function in the working, social, living and learning environments of their choice and to prevent further recurrences of homelessness by providing training and support.

PRIMARY TASKS & RESPONSIBILITIES:

1. Maintain primary responsibility for an individual caseload of residents.

2. Assessment of residents in terms of functional abilities and service needs.

3. Develop service plans, with resident input, aimed at assisting residents establish and accomplish personal goals.

4. Provide necessary linkage to needed/desired services (e.g. day programs, vocational rehabilitation, legal services, medical/dental services, outpatient psychiatric services) and attend case conferences focusing on coordination of treatment/service provided to residents.

5. Assist/teach residents daily living/social skills including, but not limited to, budgeting, personal hygiene, maintenance of living space, shopping, traveling, meal planning/preparation and constructive use of leisure time.

6. Facilitate resident participation in Community Meetings, Tenant’s Association and Social Hour.

7. Assist residents to become independent in medication management including medication supervision, education about purposes and side effects of medication and monitoring/documenting residents’ medication regimens.

8. Participate in weekly staff meetings and supervisory sessions with Program Director.

9. Maintain case records and other documentation in accordance with agency policies and procedures.

10. Assist residents in obtaining and maintaining all benefits to which they are entitled.

11. Flexibility to provide program coverage which deviates from regular schedule to meet resident and program needs.
