Job Summary
We are seeking a dedicated and highly skilled Case Manager to coordinate and oversee a case load of senior individuals who need help with money management and coordination of social services. The ideal candidate will possess strong communication skills and timely documentation completion. As a Case Manager, you will serve as a guide toward financial stability. This individual will also be part of an administrative team as well as possible help implement a new program called Guide for seniors with dementia.
Duties
- Conduct comprehensive assessments to determine individual needs and develop personalized care plans.
- Coordinate automatic bill pay when needed.
- Organize mail and assure money is managed in a participant driven format.
- Collaborate with fellow social workers and staff to assure insurance and other areas of the individuals needs are met.
- Utilize Quicken for data entry and tracking. WORD for case noting.
- Identify and assess the needs of those with dementia, while working with their care givers to coordinate services and check in monthly to monitor and modify as needed.
- Learn all of the Prairie Council on Aging Programs to service participants fully.
Skills
- Effective communication skills for collaborating with multidisciplinary teams including social workers and administrative team.
- Time Management to complete documentation and work with a monitor as needed for clarity.
- Ability to Collaborate with fellow staff member, community resources and family members to assure all options are explored for the betterment of participants.
Pay: $35,000.00 - $38,000.00 per year
