Case Manager


Job purpose


The Servant Center’s Medical Respite program is a vital bridge between hospital care and stable housing, offering a safe, compassionate environment where 43 unhoused adults can heal with dignity after a health crisis. Residents are supported not only in their physical recovery, but also in reconnecting to health care, community resources, and pathways toward greater stability and independence.

The Case Manager plays a key role in advancing this mission by providing intensive case management, helping the clients to improve their health and to address barriers to housing.

The program serves both veterans (21 beds) and non-veterans (22 beds) experiencing homelessness who have medical needs and can independently complete activities of daily living (ADLs). 

The estimated case load is 15-20 residents.


Essential Functions


Resident Care

  • Facilitates goal setting with the residents by assessing individual needs, strengths, barriers, and readiness to change.
  • Provide referrals for services, including benefits, mental health, substance abuse, employment, housing, and more.
  • Assist with applications for vouchers, housing, SNAP, SCAT, and other services/benefits as needed to address barriers.
  • Works collaboratively with treatment teams and community partners to improve resident care.
  • Participate in weekly treatment team meetings.
  • Assist in the coordination of transportation.
  • Assist in the coordination of social activities.
  • Accompany residents to appointments as needed.


Meetings

  • At least weekly meetings with residents for case notes, progress, and goal setting.


Data Tracking/Administration

  • Maintaining client records and entering data into HMIS.

Qualifications


  • Valid North Carolina Driver’s License
  • Ability to be added to The Servant Center’s motor vehicle insurance policy.
  • CPR/First Aid Certification
  • Minimum of two (2) years of case management experience.
  • Minimum of two (2) years working with the homeless or other vulnerable populations
  • Experience working with those with substance abuse and/or mental health diagnoses.



Working Conditions


Location:   1915 Boulevard Street, Greensboro, NC

Schedule:   4 – 10-hour shifts a week and one weekend a month; rotating on-call


  • Most work is performed in a residential setting in an office.


Physical Requirements


Required frequent sitting for long periods, operation of standard office machines, and computer.

Lifting to 25 pounds may be required occasionally.
