We are seeking a compassionate and motivated Case Manager to join our Substance Use Disorder (SUD) Clinic team. In this role, you will provide essential support and care coordination services to individuals seeking treatment for substance use disorders within the community. This position helps ensure accessible, high-quality care for clients during standard service hours.
Conduct comprehensive assessments of client needs, including substance use history, mental health status, and social determinants of health.
Develop individualized treatment and service plans in collaboration with clients, aligned with their recovery goals.
Coordinate and facilitate referrals to appropriate services, including detoxification, counseling, medical care, peer support, housing, and community resources.
Monitor client progress and engagement in services, providing ongoing support, advocacy, and crisis intervention when necessary.
Complete regular follow-ups to ensure adherence to treatment plans and assist with removing barriers to recovery.
Maintain accurate, timely, and compliant documentation in accordance with Indiana state regulations, Medicaid requirements, and agency policies.
Collaborate with a multidisciplinary team (physicians, therapists, peer recovery specialists, and other providers) to ensure coordinated care.
Remain current with best practices, regulatory updates, and evidence-based approaches in substance use disorder treatment.
Experience in substance use disorder, behavioral health, or community-based case management strongly preferred.
Familiarity with local Indiana community resources and referral networks.
Strong communication and interpersonal skills, with the ability to engage diverse populations.
Ability to work both independently and collaboratively in a fast-paced environment.
Excellent organization, time management, and documentation skills.
Proficiency with electronic health record (EHR) systems and standard office software.
