Case Manager

Employment Status: Full Time

Reports to: Clinical Director/Clinical Supervisor

The Case manager provides clients of Cairn Recovery Resources with linkage to resources in the community to meet various needs of the client, including assessments, child care and education, clothing, food, transportation, maintenance of finances, and to act as an advocate on the client’s behalf.

ESSENTIAL JOB FUNCTIONS:

  • Provides quality substance use/abuse and mental health intervention services to Cairn Recovery Resources clients. Provides prevention, consultation and educational services to clients and community organizations as directed by supervisor.
  • Serves as liaison between Cairn Recovery Resources clients, area agencies, and other community resources to ensure availability of the most appropriate programming and services possible for Cairn Recovery Resources clients. Makes referrals to other services as necessary for mental health counseling, legal assistance, and medical needs.
  • Develops and implements a case management system with the clinical team that includes assessments of each client’s goal setting, design and implementation of a service plan, monitoring of service delivery, and gradually eliminating client’s dependence on case management.
  • Remains available for client meetings, court appearances with clients, meetings with school personnel, meetings with children’s services personnel, and other reasonable requests by clients.
  • Prepares all required case management documentation in a timely manner, to include forwarding of applicable correspondence and progress notes to collateral contacts in accordance with agency guidelines. Maintains well organized, confidential case records, and progress notes daily within own caseload.
  • Assists with the caseload of other case managers during emergencies, illness, and vacations and assists others with group case management upon request from supervisor.
  • Participates in individual and group client supervision and attends all required meetings, including Case Management Meetings, Treatment Team Meetings, Interventions, and other meetings as requested by supervisor.
  • Remains knowledgeable regarding new developments and treatment strategies in the field through attendance at meetings, seminars, and industry research.
  • Participates in Cairn Recovery Resources’ Quality Improvement activities, conducts peer reviews according to agency policy, and maintains client records in compliance with all Quality Assurance, OhioMHAS, CARF, and agency guidelines, and performs other duties as assigned.

KNOWLEDGE/EXPERIENCE REQUIRED:

CDCA licensure required

Broad knowledge of the social service system.

3-5 years work with underserved populations preferred

SKILLS REQUIRED:

Proficiency in Microsoft Office

Decision making skills

Strong oral/written communication skills

Problem/conflict resolutions

Ability to relate to and communicate with a broad socio-economic cross-section of individuals

OTHER JOB REQUIREMENTS:

Hold and maintain an Ohio driver’s license with good driving record

Required to travel on company time

Furnish a reliable, personal means of transportation to accomplish work objectives

Job Type: Full-time

Pay: $20.00 - $24.00 per hour
