Case Manager

CASE MANAGER NEEDED

Seeking a qualified energetic person to oversee and manage host homes for persons with intellectual disabilities. Minimum education - Bachelor’s Degree in human services, social work or psychology. Prefer good computer skills, people skills a must. Responsibilities include monthly visits to up to 15 host homes to review the home environment and compliance with ADSD rules and documentation. Will need to support the IDD individuals living in the homes and the families providing services. This will include working with an interdisciplinary team by preparing for and attending annual meetings, random audits, and Human Rights Committee reviews. A strong applicant will be able to communicate well with and provide info for ADSD Service Coordinators via email, text, phone and in person at meetings. Looking for someone who can develop good relationships with the IDD people receiving services. A team member who continues and improves our company’s excellent reputation with the state could be quickly promoted to Assistant Director with an opportunity to become Director.
