Community Collaboration of Delaware (CCD) is a nonprofit organization dedicated to empowering individuals and strengthening communities through recovery support, prevention, education, and advocacy. CCD operates multiple FSARR-certified recovery residences and provides recovery support services for individuals affected by substance use disorders.

Position Summary
The Case Manager provides case management, care coordination, recovery planning, and recovery support services to residents participating in CCD's CARE (Community Advocacy & Recovery Empowerment) Program. Operating within a Social Model of Recovery framework, the Case Manager develops therapeutic and recovery-oriented working relationships with residents, supporting them in identifying goals, accessing resources, building recovery capital, and progressing toward greater independence.
This position is primarily assigned to CCD's women's recovery residences. Experience working with women in recovery and an understanding of the unique needs, experiences, challenges, and strengths of women in recovery are strongly preferred.
The Case Manager serves as a primary point of contact for assigned residents and is responsible for coordinating services across CCD's recovery residence continuum, including Level 3 and Level 2 houses. Responsibilities include resident screening and admission, assessment, recovery planning, care coordination, discharge planning, facilitation of recovery-oriented and life skills programming, resident accountability, and collaboration with treatment providers, probation officers, employers, and other community partners.
Key Responsibilities
Recovery Planning and Care Coordination
· Conduct assessments, intakes, and resident orientations
· Develop, implement, monitor, and revise individualized recovery plans
· Coordinate care with treatment providers, probation officers, TASC, healthcare providers, employers, and community partners
· Assist residents in accessing housing, healthcare, employment, education, benefits, and recovery supports
· Develop discharge and transition plans that support long-term recovery
Recovery Programming and Resident Engagement
· Facilitate recovery-oriented, psychoeducational, and life skills groups
· Facilitate MORE, Living Skills, and other approved curricula
· Conduct house meetings and structured resident activities
· Promote resident engagement in treatment, recovery support meetings, employment, education, and community involvement
· Support overdose prevention and recovery education activities
Recovery Residence Operations
· Conduct room inspections and monitor compliance with house standards
· Support resident accountability and adherence to program expectations
· Administer urine drug screens and breathalyzer tests
· Respond to resident crises and behavioral concerns using trauma-informed approaches
· Transport residents to appointments and approved program activities when needed
· Assist with overdose response and administer naloxone when trained and authorized
Documentation and Compliance
· Maintain timely, accurate, and objective documentation
· Utilize electronic health record systems and other technology platforms
· Complete incident reports, progress notes, and other required documentation
· Maintain confidentiality in accordance with HIPAA, 42 CFR Part 2, and organizational policies
Qualifications
Required
· High school diploma or GED
· Valid driver's license and acceptable driving record (no more than two points)
· Ability to transport residents using CCD vehicles
· Ability to pass criminal background screening and drug screening
· Ability to work effectively within a structured residential environment
· Strong communication, organizational, and interpersonal skills
· Ability to maintain professional boundaries while building rapport with residents
Preferred
· Associate's degree in human services, behavioral science, or related field
· Experience providing case management, recovery support, behavioral health, or substance use disorder services
· Certified Peer Recovery Specialist (CPRS)
· Experience facilitating groups
· Experience working in recovery housing or residential settings
· Experience working with women in recovery
· Minimum of two (2) years of continuous recovery from a substance use disorder
· Active participation in community-based recovery supports and a demonstrated commitment to maintaining personal recovery.
· Ability to serve as a positive recovery role model for women participating in residential recovery services.
