SUMMARY
Counsels and aids children in care requiring foster placement through Bridges to Permanency by providing case management services.
ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING

emotional, physical, and environmental well-being through phone contacts, email and
initial home visits.
treatment such as behavior modification, stress management, and therapy to assist
client in controlling disorders and other problems.
relapse, hospitalization, placement disruption, and other negative outcomes, while onsite.
including providing awake, continuous coverage throughout the night to meet the
particular needs of a child if needed;
concerning treatment plans and make recommendations as needed.

SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
QUALIFICATIONS
The Case Manager shall possess experience in working with children and families. The ideal
candidate will have excellent writing skills for case notes. The Case Manager should be able to develop trust with families in order to assist them with effective care for children in their home.
EDUCATION, LICENSURE and/or EXPERIENCE:
The Case Manager shall possess a Bachelor’s degree in a Human Services field with one year of related experience.
LANGUAGE SKILLS
Ability to read, analyze, and interpret professional journals and general business periodicals.
Ability to write reports, presentations and business correspondence.
Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information and respond to questions from groups of clergy, parishioners, managers, clients, customers, and the general public.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
OTHER REQUIREMENTS
Valid Texas driver's license, appropriate insurance and reliable car.
Must submit to and pass a pre-employment drug test.
Must submit to pass random drug tests during employment.
Must successfully pass all background screens as required by the state.
