Case Manager

About the Role

The Area Agency on Aging team is seeking caring, compassionate Licensed Social Workers (LSW) and Registered Nurses (RN) to join us as Case Managers. In this role, you will assist older adults who need long-term care services to remain safely and comfortably in their community.

Key Responsibilities

• Develop and coordinate individualized care plans to support community living.
• Organize and facilitate access to appropriate long-term care services.
• Perform home visits to evaluate client environments and provide support.
• Maintain clear, accurate documentation and communication with clients, families, and service providers.
• Collaborate with a supportive network of supervisors and co-workers to ensure quality care.

Qualifications

• Active and valid license as a Registered Nurse (RN) or Licensed Social Worker (LSW) in the State of Ohio.
• Strong assessment skills with the ability to evaluate client needs effectively.
• Excellent communication and organizational skills.
• Genuine desire to help others and make a positive impact in the community.

Work Schedule

Full-time, Monday through Friday. No weekends, evenings, or holidays.
