Position Description:

The Case Manager is responsible for coordinating services, identifying and maintaining resources that support client success, and providing ongoing case management to youth engaged in Eddy House programs. This role develops care plans, supports client accountability and progress, and coordinates with internal staff and community partners to ensure consistent, effective service delivery. The Case Manager also plays an active role in daily program operations by directing workflow, supporting staff follow-through, and using independent judgment to address client and program needs in coordination with leadership.

Essential Job Functions may include the following or other duties as assigned:

  • Serve as a primary advocate for clients and support progress through referrals, transportation, advocacy, and other services.
  • Complete case plans and related follow-up steps in a timely manner.
  • Meet regularly with clients to review goals, address barriers, and support accountability.
  • Assist with client de-escalation, stabilization, and problem-solving as needed.
  • Support client movement through Eddy House programs and help coordinate next steps.
  • Maintain timely, accurate, and professional documentation in Apricot, HMIS, and other required systems.
  • Participate in staffing, case conferencing, and multidisciplinary meetings with organized updates and recommendations.
  • Coordinate services with community partners and maintain positive working relationships with outside providers.
  • Communicate regularly with clients, service providers, and team members to support continuity of care.
  • Assign and direct day-to-day work of Client Services Specialists and other relevant staff related to client support, documentation, appointments, transportation, and program transitions.
  • Provide guidance to staff regarding workflow, program expectations, and service delivery.
  • Support onboarding, training, and day-to-day accountability of Client Services Sepcialists and other relevant staff.
  • Review staff concerns related to program operations, gather context, make recommendations, and support implementation of responses in coordination with leadership.
  • Communicate recommendations to leadership regarding staff performance, corrective action, hiring, and recognition based on observed performance and program needs.
  • Provide education, presentations, and training to community partners and Eddy House staff as assigned.
  • Maintain professional, client-focused, and effective communication.
  • Assist others as requested and perform related duties as required.
  • Maintain a safe, clean, and professional work environment.
  • Maintain a professional and courteous appearance and demeanor.

Requirements/Qualifications

  • Required: Ability to pass a background check.
  • Valid Nevada driver’s license and reliable vehicle with current vehicle registration and insurance.
  • Minimum: Bachelor’s degree or higher in Social Work, Psychology, Criminal Justice, Public Health, Human Development and Family Studies or related field.
  • Previous experience as a Case Manager for at-risk youth, preferred.
  • Must demonstrate high level of confidentiality and professionalism.
  • Proficient with Microsoft Suite and high degree of computer literacy.
  • Strong interpersonal skills with the ability to communicate clearly and in a calm and pleasant manner.
  • High level of organization and attention to detail with demonstrated interpersonal skills.
  • Ability to read and write checklists, reports, and correspondence. Ability to read and comprehend complex instructions, memos, etc.
  • Ability to analyze and perform complex computations with reasonable speed and accuracy. Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

While performing the duties of this job, the employee is regularly required to sit, use hands, stand, walk, bend, drive, and have specific vision abilities to include close and distance vision, and ability to adjust focus working with computer business equipment and while driving. Must be able to lift up to 25 pounds on occasion. The noise level in the work environment is usually low level.

The overall nature of the position is sedentary requiring little physical effort and occasional light exertion. There is occasional exposure to environmental conditions such as heat, cold, and temperature changes.

Pay: $45,000.00 - $50,000.00 per year
