Position Title: Case Manager

Reports To: Executive Director

Position Summary: The Case Manager will be responsible to assist clients and potential clients in assessing barriers to housing and developing a plan to overcome those barriers.

The Case Manger will be responsible to develop and maintain relationships with landlords, employers, community service agencies and others key to providing effective client management.

The Case manager performs a wide range of administrative and communication support activities while working with a diverse client population.

The Case Manager will be required to participate in planning and executing shelter program delivery.

The Case Manager will be responsible to support the execution of program grants.

The Case Manager will be part of the shelter services team required at times to provide front desk coverage to the Day Shelter.

The Case Manager serves in a professional capacity in the area of client services to include four major categories:

HOUSING SERVICES — Maintain data related to Emergency Housing Assessment — Help to identify needs of clients and access appropriate support services that enable program participants to stabilize their housing — Work directly with clients on solutions to prevent future homelessness

RESOURCE MANAGEMENT — Develop and maintain relationships vital to service delivery —Develop and maintain resource list for ready referral — Identity and develop new and changing resources — Maintain a landlord database for housing options — Research and post housing options for shelter clients — Ensure accurate data collection — Maintain accurate client files

PROGRAM DELIVERY — Identify and develop client skill building events — Plan and execute client outreach events — Direct volunteers, as needed when volunteer coordinator is not available, in shelter practice and policy

OTHER DUTIES AS ASSIGNED

POSITION QUALIFICATIONS Associate degree or equivalent, with preference given to a bachelor’s degree in a social services, human management, or equivalent experience. Previous experience in volunteer or asset management preferred Previous experience working with service organizations

SKILLS ABILITIES AND REQUIREMENTS Excellent communication skills, both written and verbal. Strong analytical, critical thinking, and problem-solving abilities. Strong collaboration and team skills. Great organizational and time management abilities. Computer Skills: High level of ability with Microsoft Office applications (MS Word, Excel, Outlook), design software and ability to learn shelter and grant specific software This position may require some travel across the region served by GUEST, reliable transportation is required to fulfill the essential functions of the position This job will require Case Manager to work some evenings, weekends and holidays. This position is currently operating at 40 hours per week December to March and 20-30 hours per week April to November.

Pay: $20.00 - $25.00 per hour
