Case Manager

Position Purpose: The Case Manager is responsible for coordinating comprehensive, individualized, and trauma-informed case management services that support the emotional, medical, legal, vocational, social, and aftercare needs of clients in residential and outpatient behavioral health programs. The Case Manager serves as a central point of communication and resource linkage across the interdisciplinary treatment team, external providers, and families. This position is integral to treatment continuity, discharge planning, and client success beyond the program.

All duties are to be performed in accordance with Florida DCF Chapter 65D-30 standards, AHCA requirements, Joint Commission accreditation guidelines, HIPAA/42 CFR Part 2 privacy laws, and internal Remedy Therapy policies and procedures.

Qualifications/Position Requirements:

1. Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field preferred; however, equivalent industry experience and demonstrated job knowledge will be considered in lieu of formal education.

2. Experience: Minimum one (1) year of experience in behavioral health, social services, or case management (SUD and/or ED experience strongly preferred).

3. Certification/Licensure: Clear and valid driver license required. Acquire within 6 months of hire and maintain CPR/AED/First Aid Certification. CBHCM (Certified Behavioral Health Case Manager) certification preferred but not required.

4. Training and Knowledge Required at Time of Hire: Working knowledge of DCF 65D-30, AHCA, and Joint Commission standards related to case management and behavioral healthcare. Understanding of SUD and ED recovery models, co-occurring disorders, trauma-informed care, and client-centered service delivery. Proficiency in EMR systems (e.g., KIPU), Microsoft Office, and secure communication platforms. Ability to multitask, prioritize, and maintain professionalism in a fast-paced environment.

5. Age Specific Individuals Served/Responsibility: Adults (18+ years)

6. Security Measures and Clearance: Approved Level 2 screening required. Local Background may also be required.

7. Employment Classification: Hourly/Non-exempt. Part-time/Full-time/Per diem.

Position Specific Competencies:

1. Conduct a comprehensive Initial Needs Assessment with each client within 24 hours of admission

2. Coordinate and document all appointments with outside providers, including primary care, psychiatry, endocrinology, specialty medical providers, legal, and other behavioral health resources

3. Begin discharge planning upon admission; coordinate individualized aftercare plans in collaboration with the client, therapist, nursing, medical, and family (if applicable)

4. Conduct weekly check-ins with assigned clients to assess progress, address barriers, and update recovery goals

5. Know and maintain professional communication skills: verbal, written, and non-verbal communication

6. Understand and maintains the highest level of HIPAA (Health Insurance Portability and Accountability) standards

Position Duties, Responsibilities, and Tasks:

1. Core:

a. Identify barriers to treatment engagement and recovery, including legal, vocational, housing, family, transportation, and medical issues.

b. Develop and document individualized Case Management Service Plans, including short- and long-term goals with measurable objectives.

c. Update service plans at minimum every 30 days or as clinically indicated.

d. Arrange and monitor FMLA, short-term disability, and medical leave documentation when appropriate, working closely with HR representatives, clients’ employers, and physicians.

e. Assist clients in applying for benefits and entitlements, including Medicaid, Social Security, SNAP, housing, and transportation services.

f. Facilitate referrals for supported employment, vocational rehabilitation, and educational programs as needed.

g. Support clients during moments of distress through brief therapeutic interventions, problem-solving, and emotional de-escalation as appropriate within scope.

h. Secure discharge appointments with outpatient providers, including psychiatry, therapy, primary care, nutrition/dietician, or support groups.

i. Coordinate admission to step-down programs (e.g., PHP, IOP, sober living, supportive housing, residential eating disorder programs) as appropriate.

j. Complete detailed Case Management Discharge Summaries including all aftercare referrals, contact info, and follow-up expectations.

k. Coordinate and attend treatment team meetings; present updates on case management needs and progress.

l. Communicate and collaborate with external agencies such as probation officers, guardians, outpatient providers, courts, or child welfare.

m. Facilitate family communication and involvement, including organizing family meetings, educating on discharge needs, and maintaining proper releases.

n. Ensure all case management notes, assessments, progress updates, and contact logs are entered in the EMR (e.g., KIPU) within 24–48 hours of service.

o. Maintain compliance with Joint Commission, DCF, and AHCA standards, including record keeping, audits, and performance improvement initiatives.

p. Follow all HIPAA and 42 CFR Part 2 confidentiality regulations.

q. Serve as a liaison between departments to ensure timely information exchange between clinical, medical, nursing, and admissions teams.

r. Support nursing and clinical staff in coordinating care transitions and urgent needs such as hospitalizations or emergent outside consults.

s. Participate in utilization review meetings as requested to report on case management involvement and barriers to discharge.

Pay: $22.00 - $26.00 per hour
