Case Manager – Outpatient Substance Abuse & Mental Health Services

Location: Peak Recovery

Employment Type: Full-Time or Part-Time Available

Position Summary

The Case Manager provides support and resource coordination for individuals receiving outpatient treatment for substance use disorders and co-occurring mental health conditions. This role focuses on helping clients overcome barriers to recovery by connecting them with community resources, coordinating care, and supporting treatment goals.

Responsibilities

Conduct client needs assessments and assist with service planning.

Coordinate care with treatment providers, medical professionals, probation officers, employers, schools, and community agencies.

Assist clients with obtaining:

Housing resources

Employment assistance

Transportation services

Medical and mental health services

Insurance and benefits

Educational and vocational opportunities

Community support services

Provide advocacy and support throughout the treatment process.

Assist clients in identifying and overcoming barriers to recovery.

Maintain accurate and timely documentation.

Participate in treatment team meetings and collaborate with clinical staff.

Build positive relationships with community partners and referral sources.

Qualifications

Preferred:

Bachelor's degree in Social Work, Psychology, Human Services, Behavioral Health, or a related field.

Experience working with substance use disorders, mental health, or social services.
