Case Manager ACT

JOB SUMMARY

The ACT MHP (Mental Health Professional) Case Manager is responsible for providing comprehensive, community-based services to adults living with serious mental illness, utilizing the Assertive Community Treatment (ACT) model. This position focuses on client engagement, recovery-oriented care, coordination of services, and ongoing support to help individuals achieve stability and independence within the community.

JOB DUTIES

  • Complete 6–7 billable client engagements daily
  • Participate in a 24/7 on-call rotation for crisis intervention
  • Conduct biopsychosocial assessments and develop IM+CANS documentation
  • Coordinate services across housing, medical, vocational, and mental health domains
  • Provide outreach, supportive counseling, and problem-solving strategies
  • Collaborate with internal teams and external agencies for discharge planning
  • Ensure all documentation meets Medicaid, agency, and state standards
  • Maintain accurate and timely DIS billing for services rendered
  • Promote mental health awareness through education and outreach

QUALIFICATIONS

  • Bachelor’s degree in Social Work, Psychology, Counseling, or another closely related social science field is required.
  • Master’s degree preferred.
  • Two years of experience in community or behavioral health preferred
  • Valid Illinois Driver’s License, reliable transportation, and ability to travel within service area
  • Familiarity with DSM-5, ASAM, and Medicaid-funded programs
  • EHR proficiency and strong communication/documentation skills
  • Culturally competent; bilingual (especially Spanish) preferred

  • Client Stability: 90% of assigned clients maintain housing and avoid hospitalization
  • Treatment Compliance: 100% of treatment plans completed on time with measurable goals
  • Resource Utilization: Build relationships with 3 new community resources quarterly

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

  • Travel throughout the community required
  • May require lifting 10–25lbs, walking/standing for extended periods
  • Minimal physical risk

COMPENSATION & BENEFITS

Salary: 52,500

Occupational Risk for Exposure to TB

This position involves direct patient contact, posing a risk of exposure to Tuberculosis (TB). Employment is contingent upon annual TB testing, except for:

  • Documented medical contraindications
  • Documented religious objections

Staff who decline TB testing or flu vaccinations due to medical or religious reasons will be required to wear a mask as appropriate.
